How you make a difference

As a Recovery Plan Coordinator, you willbe responsible fororganizing, coordinating,facilitating, andmonitoringthe provision of all treatment and rehabilitative services for patients in assigned patient living areas. Your primary goal is to help patientsacquireand use the skills andsupportsnecessary to achieve maximum independence, success, and satisfaction in their preferred environment. This includes initiating andmaintainingRecovery Plansin accordance withall facility policies, protocols, and guidelines.

Key Responsibilities

  • Coordinate and facilitate the development of Recovery Plans and associated documentation, and schedule Treatment Team meetings to review and update plans on a monthly basis.

  • Advocate for and make referrals for individualized services based on assessed need,monitorpatientparticipationand progress, and conduct monthly visitation reviews.

  • Serve as an expert resource on psychiatric rehabilitation,providetraining to team members, anddemonstratecultural and age-specific competency in all duties performed.

  • Cooperate with colleagues to make decisions in the best interests of patients and contribute to improving facility service delivery.

  • Ensure all duties are performedin accordance withfacility policies and guidelines.

Qualifications & Requirements

Education

  • Bachelor’s Degree in a human services field required. Master’s degree preferred.

Experience

  • (3) years’ applicable experience in a mental health setting is preferred.
