How you make a difference

As a Rehabilitation Specialist II, you'll play a pivotal role in developing, organizing, and facilitating Group Activities on your assigned unit, collaborating closely with the treatment team to align with the interdisciplinary treatment plan set forth by Wellpath Recovery Solutions. Your primary responsibility will be to provide daily Groups/Activities aimed at aiding individuals grappling with mental and emotional challenges, as well as addressing relationship issues such as trust and self-esteem, fostering an environment conducive to holistic healing and rehabilitation.

Key Responsibilities

  • Plan and facilitate engaging group activities, skillfully managing group dynamics to optimize participation, while documenting monthly changes and reporting them to the team through notes and meetings.

  • Conduct monthly reassessments to gauge readiness for progression, meticulously documenting results in notes. Demonstrate efficiency in completing various documentation tasks within specified time frames, including progress notes, admission assessments, and monthly re-assessments.

  • Contribute to the planning of special events by collaborating with departments and programs, ensuring alignment with the diverse needs of individuals receiving services.

  • Display sensitivity to cultural diversity, treating each person served as an individual and incorporating cultural considerations into care and treatment. Tailor individualized care to align with age and developmental needs.

  • Adhere to CCRS Code of Conduct Standards, applying critical thinking principles to practical and emergent situations. Demonstrate the ability to exercise sound judgment, pay attention to detail, work under stress, and perform additional assigned duties as required.

Qualifications & Requirements

Education

  • Bachelor’s degree in a human services field required. Degree requirement for employees hired after 2010.

Experience

  • One-year experience in a mental health setting.

Licenses/Certifications

  • None required.
